Pay Range:

$39.60 - $59.40

A successful candidate's actual pay rate will be based on several factors including relevant experience, skills, training, certifications and education.

The Physical Therapist II–Home Care completes comprehensive physical therapy assessment in patient's home. Establishes treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility. The Physical Therapist II–Home Care works cooperatively with physicians, case managers, and adjustors. Documents and reports on patients' progress. Additionally, Physical Therapist II–Home Care monitors, documents, reports patients' progress and serves as an educator for patients, families, students, co-workers, and self-professional advancement.

Core Function: Physical Therapy
Schedule: Full-Time, 40 hours/week / weekend rotation (every nine weeks)
Facility: St. Anthony’s Hospital
Location: Belleville, IL Service Area
Sign On Bonus: Up to $10,000 for external candidates
Compensation: Compensation that aligns with your experience
Education Qualifications
  • Graduate (e.g., bachelors, masters, or doctorate) from an accredited Physical Therapy program which meets the licensure requirements in the state of practice is required.
Experience Qualifications
  • 2 years of Physical Therapy experience is required.
Certifications, Licenses and Registrations
  • Illinois: Licensure or Eligibility for licensure as a registered physical therapist by the State of Illinois under the condition that they have been issued a letter of authorization from the Illinois Department of Financial & Professional Regulation (IDFPR) allowing them to practice under supervision is required.
  • Wisconsin: Licensure or Eligibility for licensure as a registered physical therapist by the Physical Therapy Examining Board in Wisconsin under the condition that they have been issued a temporary license allowing them to practice under supervision is required.
  • Certified Basic Life Support (BLS) or completion in the first 90 days of employment required.
  • Valid state driver’s license, current automobile insurance and reliable automobile is required.
  • Board Certification (e.g., CHT, OCS, etc.) which meets rigorous standards of maintenance or certification is preferred.
